New Customer Questionnaire
Thank you for your interest in SeQure Underwriters. Prospective
customers are encouraged to fill out our New Customer
Questionnaire, below. After filling out the basic questions,
someone will be contacting you to speak further. You will be asked
to provide the following documentation regarding your program:
- 5 years of currently valued loss history
- 5 years of limits of coverage
- Copy of current desired policy forms as well as any form
changes needed
- 5 years of current retentions or deductibles
- Copy of sample underwriting guidelines
